On the front left tail of the Hornacek road jersey is the "Champion" manufacturer's tag with wash instructions and jersey size "44" on it. Adjacent is another Champion "90" exclusive" tag with "BODY LENGTH 0" on it. Across the front of the jersey is "PHOENIX" in orange on white tackle twill. Below that and on the back of the jersey is the player number "14" in the same tackle twill. Across the back of the jersey is "HORNACEK" in white tackle twill. Inside the waistband of the Hornacek road shorts is the "Champion" manufacturer's tag with wash instructions and jersey size "34" on it. Adjacent to that is another Champion "90" exclusive" tag with "RISE +2" on it. "14" is written on both tags in black marker. In the back collar of the Johnny High jersey is the "Spanjian" size 42 manufacturer's tag. Across the front of the jersey is "PHOENIX" in orange on purple tackle twill. Below that and on the back of the jersey is the player number "22" in the same tackle twill. Across the back of the jersey is "HIGH" in purple tackle twill. Walter Davis Converse manufactured white sneakers with purple accent colors. "SWEET D" is screen printed in purple on the side of each shoe. Both sneakers are autographed in blue marker and rate an 8. The uniform, jersey, & shoes all show game-use and appear to have no alterations of any kind. The autographs are accompanied by a LOA from JSA.
