On the back inside collar is the size 58 "Made in Canada" tag. On the front is the large "Avalanche" logo patch embroidered in burgundy, blue and black on white. On the back and on both sleeves is the player number "4" in white on blue on silver triple tackle twill. Across the back is the player name "BLAKE" in white tackle twill on a burgundy name plate. Above the name on back is the "KOHO" embroidered patch. On each shoulder above the number is the "Avalanche" logo patch embroidered in burgundy, blue, and silver on white. On the lower right back is the "NHL" embroidered logo. On the inside back is the original fight strap. On the front left breast is the Alternate Captains "A" patch in white on blue on silver triple tackle twill. Inside the back tail is a tag which reads "NHL Game Worn Jersey 2003-2004 Season B11407." The jersey shows wear and appears to have no alterations of any kind. Accompanied by an LOA from Milt Byron.
