The bottom of the jersey has been custom hemmed, shortened, and elastic has been added with no tags remaining. On the inside back collar of the jersey is a tag with "04-50". On the front, back, and on each shoulder is the player number "52" in white on gold on black triple tackle twill. Across the back is "R.LEWIS" in white tackle twill on a purple name plate. On the front at the neck is the official "NFL Equipment" embroidered logo patch. Embroidered below that is "RAVENS". On each sleeve below the player number is the "Reebok" embroidered logo and below that is the "RAVENS" crest patch. The jersey has purple spandex sides, custom elastic sleeves, and a custom elastic neck. On the left chest is the Ravens "X Anniversary” patch. The jersey is tagged from 2004 but worn in 2005. The jersey shows heavy game-use and besides being customized to the player’s specifications appears to have no alterations of any kind.
