On the front left tail of the jersey is the "RUSSELL ATHLETIC" manufacturer's tag with wash instructions and a flag tags attached reading "50, 100% POLYESTER," “+2” EXTRA LENGTH” and “+2” SLEEVE LENGTH.” Below is the “2004” strip tag. Across the front of the jersey is "CLEVELAND". On the back is player number "41" and across the back of the jersey is "MARTINEZ" sewn on a grey name plate. All lettering and numbering is done in red on silver on navy blue triple tackle twill. On the back of the collar is the official “MLB” logo patch. On the left sleeve is the "INDIANS" team logo patch with the “RUSSELL” embroidered logo below. On the right sleeve is the “AEON JAPAN ALL-STAR SERIES 2004” patch. Autographed on front and back in blue marker and rates a 9. The jersey shows game use and appears to have no alterations of any kind. The autograph is accompanied by a LOA from JSA.
