On the left front tail is the "Medalist Sand-Knit" manufacturer's size 40 tag. Above is the "Designed & Tailored Exclusively for the Denver Nuggets" tag. Adjacent is an additional manufacturer's embroidered tag with “2” L.B.” on it. On the front is "NUGGETS" with the player number "12" above on the left breast. On back is the player number “12” in blue on white tackle twill. Across the back is the player name "LEVER" in blue tackle twill on a white name plate. On the upper left breast is the official "NBA" embroidered logo. The jersey is autographed on front in black marker and rates an 8. The jersey shows game-use and appears to have no alterations of any kind. The autograph is accompanied by a LOA from JSA.
