In the back collar is the "Spanjian" manufacturers size 44 tag. Attached is a wash instructions flag tag. Across the front is "PHOENIX" in orange on white tackle twill. Below that and on the back is the player number "45” in the same tackle twill. Across the back is the player name “COOK” in white tackle twill. The jersey shows game-use and appears to have no alterations of any kind. Accompanied by a MEARS A10 LOA.
