On the right front tail is the "REEBOK" manufacturer's tag. Attached to that are four additional manufacturer's tags with the jersey size "48," "+4 LENGTH," "2002-2003" and “PLAYDRY” on them. Above that is an additional tag "Hardwood Classics" with a "1968-69" year tag. Across the front of the jersey is "PHOENIX." Above that and on the back is the player number "23." Across the back is the player name "JACOBSEN" in orange on purple tackle twill. On the upper left breast is the embroidered official NBA logo. Autographed on the back number in black marker and rates a 9. The jersey shows game-use and appears to have no alterations of any kind. The autograph is accompanied by a LOA from JSA.
