On the front left tail of the jersey is the "Russell" size 50 manufacturers tag with wash instructions. To the left is the “81” embroidered paper year tag. On the front, back and each shoulder is the player number "57" in grey on black screen print. Across the back is the player name "VAN NOTE" in white tackle twill on a red name plate, which has been restored. Below the numbers on each sleeve is the “FALCONS” logo in black and grey screen print. The jersey shows heavy game-use and besides the name plate being restored appears to have no other alterations of any kind.
