Approximately 35” x 22” framed display of a Jack Dempsey referee application. There are two photos of Jack Dempsey with a “State Athletic Commission of California REPORT OF PHYSICAL EXAMINATION OF APPLICANT FOR LICENSE”. The report includes Dempsey’s information including name, ring name, age (63), race, weight and address. The “HISTORY” and “EXAMINATION” fields are filled out. The bottom left of the report includes the date, 4/4/1957, and is signed by Dempsey with his full name, William Harrison Dempsey in blue pen rating a 9. Beneath the report is a plaque that reads “JACK DEMPSEY - THE MANASSA MAULER”. Accompanied by an LOA from JSA.
