6 ½” x 3 ¾” Apollo 15 NASA issued astronaut insurance cover. On the front of the envelope is the NASA “APOLLO 15” logo with the three astronauts last names “SCOTT”, “WORDEN” and “IRWIN” wrapped around the bottom of the logo. Stamped by “KENNEDY SPACE CENTER, FL JUL 26 AM 1971”. The cover is signed by all three astronauts under their name in the logo in black marker, in our opinion the signatures rate an almost perfect 10 overall. On the back of the envelope is “Insurance Cover” written in black ballpoint pen. Adjacent below is “From My Personal Collection - Al Worden - Apollo 15 CMP” written in black marker. Under the envelope seal reads “#152amw” written in black ballpoint pen. Inside the envelope is a card with the “APOLLO 15 MISSION PROFILE” with the “FLIGHT CREW” and a brief description of each member. The insurance cover is in NM-MT condition. Accompanied by a LOA from JSA and an additional LOA from Alfred Worden, the “Command Module Pilot” on Apollo 15.
