On the front left tail is the Champion manufacturer’s tag size “44” with washing instructions. Adjacent is an additional tag that reads “DESIGNED & TAILORED 95-96 EXCLUSIVELY FOR UTAH JAZZ BODY LENGTH + 2”. On the left inseam is a materials tag. On the upper left chest is the official “NBA” logo embroidered. Across the chest reads, “UTAH JAZZ” screen printed in purple, yellow, and green. The player number “14” appears on both the front and on the back. These are screen printed in purple. Across the back reads the player name, “HORNACEK” in purple tackle twill sewn directly onto the white mesh jersey. Hornacek signed the front of the jersey in black and in our opinion the signature rates an 8. There is clear evidence of prior framing including glue and residue throughout the jersey including all throughout the sleeves, the tail and back number however the jersey displays well from the front. This jersey is properly tagged, was presented to us as game-used and in our opinion shows light use. Accompanied by a ball boy LOA and an ALOA from JSA.
